Fauna Record Form	01/07/2008		This form is to be completed for every animal in care. The form must remain with the carer whilst the animal is in care and is only returned to the record keeper once the animal's fate is known (i.e.. released, died, euthanized etc.) Please complete all details as accurately as possible.
